Welcome to the Oregon Physical Therapist Licensing Board
The Physical Therapist Licensing Board was created in 1971 to regulate the practice of physical therapy in Oregon. The Board's purpose is public protection and to establish professional standards of practice which assure that physical therapists and physical therapist assistants are properly educated, hold valid/current licenses, practice within their scope of practice and continue to receive ongoing training throughout their careers. Physical therapy practice is governed by state statutes and rules, which define the scope of practice. The Board issues licenses, promulgates rules, monitors continuing education, investigates complaints, issues civil penalties for violations and may revoke, suspend or impose probation on a licensee or limit his/her practice. The Board is comprised of seven volunteer members: five physical therapists, one physical therapist assistant, and one public member. Each member is appointed by the Governor and may serve a four-year term,with a maximum of two terms. The Board is self-supporting and activities are financed solely from licensure and related fees.

This site does constitute a "primary issuing source" for purposes of license verification. Unless otherwise noted on an individual document, the Board grants permission to copy and distribute documents and information verbatim for non-commercial use.
Mandated State Furlough Closure Dates
Due to State budget cuts, the Oregon Physical Therapist Licensing Board will observe the following mandatory closure dates:

Friday October 16, 2009; Friday November 27, 2009; Friday March 19, 2010; Friday April 16, 2010; Friday June 18, 2010; Friday August 20, 2010; Friday September 17, 2010; Friday November 26, 2010; Friday March 18, 2011; Friday May 20, 2011.

The Board office will resume operations on the next business day following the closure.
IMPORTANT INFORMATION
ON-LINE LICENSURE RENEWAL
You now have two options for renewing your license, you may renew using your paper renewal form or by using the online renewal system:
ELECTRONIC RENEWAL - If you renew on-line, you must pay on-line with a credit card, debit card or electronic check. You can not renew on-line if you are going to use a written check for payment.

To use the on-line system you must also have your unique renewal indentification number found in the top right hand corner of your paper renewal form and you must know the last four digits of your social security number.

PAPER RENEWALS
Paper Renewal - You can renew by mailing your completed renewal form back to the Board at P.O. Box 5185, Portland, OR 97208-5185 along with a written check. The Board can not accept an electronic payment with a paper renewal form.

Renewal forms will be mailed during the week of January 11, 2010. If you have not received your paper renewal form by the end of January, contact the Board office at 971.673.0201.

PAIN MANAGEMENT CONTINUING EDUCATION
If you were initially licensed prior to 4/1/2008, as part of your 24 hours of CE you must have completed the mandatory 1-hour on-line presentation by the Oregon Pain Commission (see link below) by March 31, 2010.

Additionally, as part of your 24 hours, you must also complete 6 hours of CE relative to the management or treatment of pain. This is a one-time legislative requirement. PTAs are not required to meet this mandate but may do so if they choose. Physical therapists licensed on or after 4/1/08 will have to complete the pain management course requirement within 24 months following their first license renewal.
